Success Story

Using Evaluation to Promote Sustainability

The SOSA Consortium is a network of organizations and individuals engaged in substance use disorder (SUD) prevention, treatment, recovery and workforce development in Santa Cruz County. The PEI worked with the SOSA Consortium for six years with three different HRSA Rural Community Opioid Response Program grants from 2019-2025. In 2025, the PEI facilitated a “shared history” activity in which consortium members created a timeline of events, collaborations, opportunities and challenges they had experienced in their community related to SUD. As the final grant period came to a close and the SOSA Consortium members had to decide if and how to move forward, we used to shared history activity to spark conversation about the SOSA Consortium’s accomplishments, strengths and impact. This discussion led to plans to continue the Consortium and focus on the activities that had been most meaningful to the members.
